Paperwork for Teams

Listed below is the necessary paperwork for NEW teams to get into a League(Softball, Volleyball, Soccer or Basketball)  All teams must turn in an application and a roster.  Individuals that are interested in joining a team are advised to show up at the mandatory league meetings in the beginning of the year or go to the Message Board on our website.  We do not place individuals onto rosters, it is entirely up to him/her to find a team to play on.
BASKETBALL
Application Agreement  This form must be filled out and mailed to our office.  This form officially puts your team into consideration for admittance into one of our four leagues(softball, soccer, volleyball or basketball).   Filling out this form will put your team on a wait list and does not guarantee acceptance into our leagues.  Some of our leagues are at capacity and this form will make you eligible for a lottery.   Form can be faxed to us at 203-977-5504.
Roster/Waiver This form is used as a roster/waiver for all leagues.  Teams are required to have all participants sign the roster/waiver. 
Add/Drop form   Paperwork for adding new players to an existing team roster.  This form must be mailed to our office with player's signature on form.  Each league has stipulations and deadlines for adding new players, so check with league officials before sending in form.  Typically, it must be in office prior to playing in game.
Referee Fee Letter:  This letter is for companies to verify how much referee's are getting paid over the course of the season.  Teams are responsible to pay the fee in cash prior to each game.
Letter to team managers:  This letter is sent to managers informing them of meeting dates and deadlines.
SOFTBALL
